HIPAA Notice of Privacy Practices

Your Information. Your Rights. Our Responsibilities. Effective: 2026.

This notice describes how medical information about you may be used and disclosed by Beacon Bridge LLC ("Beacon Bridge," "we," "us," or "our") and how you can get access to this information. Please review it carefully.

Your Rights

When it comes to your health information, you have certain rights:

  • Get a copy of your record. You can request an electronic or paper copy of your medical record. We will typically provide it within 30 days and may charge a reasonable, cost-based fee.
  • Correct your record. You can ask us to correct information you believe is incorrect or incomplete.
  • Request confidential communications. You can ask us to contact you in a specific way (for example, a particular phone or address).
  • Limit what we use or share. You can ask us to restrict certain uses or disclosures for treatment, payment, or operations. We are not required to agree.
  • Get an accounting of disclosures. You can request a list of the times we have shared your PHI, subject to legal limits.
  • Choose someone to act for you. A person with medical power of attorney or a legal guardian may exercise your rights.
  • File a complaint. You may complain to us or to the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate.

Your Choices

You may tell us your preferences for sharing information with family, friends, or others involved in your care, and in disaster relief situations. We never share your information for marketing, sell your information, or share psychotherapy notes without your written authorization.

Our Uses and Disclosures

We typically use or share your health information to:

  • Treat you and coordinate care with other providers.
  • Run our organization and improve care.
  • Bill for your services and obtain payment from health plans (including Medicaid).
  • Help with public health and safety issues.
  • Comply with the law, respond to lawsuits, and work with law enforcement.
  • Address workers' compensation and other government requests.

Substance Use Disorder (SUD) Records

SUD records created in programs subject to 42 C.F.R. Part 2 receive heightened protection. We will not disclose SUD records without your written consent except as specifically permitted by law (for example, medical emergencies, qualified service organization agreements, reporting crimes on program premises, mandated child-abuse reporting, or a valid court order).

Our Responsibilities

  • We are required by law to maintain the privacy and security of your PHI.
  • We will notify you promptly if a breach occurs that may have compromised your information.
  • We will not use or share your information other than as described here unless you tell us we can in writing. You may change your mind at any time.

Changes to This Notice

We may change the terms of this notice; the new notice will apply to all information we maintain about you and will be available on our website and in our offices.
